Responsibilities

Perform telephonic assessment of patient condition, perform telephonic coordination of member’s care with providers, coordinate interventions among all communication for providers with regard to interventions and patient status, provide communicate with contracted Health Plans to resolve issues, work with contracted vendors to assist in evaluation, intervention and coordination processes, participate with periodic audits to assure quality of work and compliance with regulatory bodies.

Qualifications & Requirements

Current California Registered Nursing License
minimum 2 year Acute Care experience or equivalent
minimum of one year Case Management experience
minimum of one year Managed Care Utilization experience
knowledge of and experience with M&R and unequal guidelines
knowledge of NCQA, Medicare and legislative regulations and strong problem solving skills
